Patients will be seen by the NICU fellow assigned to the clinic and supervised by a staff neonatologist. At the beginning of the fellowship, each neonatal fellow interacts with the program director to discuss the fellow's research interests and meets with a variety of prospective mentors. At-risk infants should be fed breast milk, and feedings should begin with small amounts that are gradually increased according to standardized protocols. Once the neonatal resuscitation team is summoned to the delivery room, it is important to obtain a pertinent history; assign roles to each team member; check that all equipment is available and functional, 1 including a pulse oximeter and an air/oxygen blender 6; optimize room temperature for the infant; and turn on the warmer, light, oxygen, and suction. The reflectance meters have correlations with the YSI ranging from r=0.88 to 0.96 for cord blood and from r=0.64 to 0.86 for capillary blood (Table 1 ⇓ ), demonstrating a fundamental shortcoming of hand-held glucose reflectance meters to determine blood glucose concentration in the neonate.
